What is it that no matter what the player, roles, meta is, DRX always chokes. Why is it so?

his timeouts have always been terrible, the timeout at 6-9 against TL was to be dreamed of though
if he breaks his binding vow the dire consequence is frost farming them for all of next year
At first its just an issue of not being disciplined or bad mental but if it goes too long without being unchecked then it engraves itself into the team and even if the players and coaches try and play it off as a coincidence or a non-issue it becomes stuck in their minds and they start to worry about it especially if its happened multiple times before and has become a narrative.
