They had to have played in any tournament on vlr, not necessarily riot organized.
No, it actually gets decided by the vlr.gg council. We the members of the community gather all weeks and decide which player is good and which player is bad. Moreover, we use that judgement to guess what stats they deserve based purely on eye test.
