from what Ik outside India roohafza sharbat is quite popular especially in Pakistan and Middle East so my guess is they developed liking for the water variant of the drink and some might even be lactose intolerant but in India most love falooda/kulfi-falooda with rooh afza
for me personally falooda/kulfi-falooda is top3 milk shake favorites
but roohafza sharbat isnt even among top10 squash favorites as there are much much better tasting options

im not muslim but rooh afza is wayy to sweet for my taste. But whenever I'm craving it, I just use water ice and a like two teaspoons of lemon juice. Its really nice. Also its the best when u swirl it on icecream

its basically rose extract, and you can have it with milk or water, I like milk better.
I think its a south asian thing, Pakistani's have it when we break fast, but I found out that Indians also have it

milk is better but it's not that much better. Rooh afza with cold water and sweet basil seeds (tukmaria seeds) is so good. you can also try it with lemonade.
